of the accommodation booking form With the exception of the Four Seasons Hotel, the accommodation rates stated are per room per night inclusive of VAT 5% and breakfast. Should VAT be increased by the government before the Conference, the new rate will be applied to the hotel price. A credit card number or bank draft issued in EUR equivalent to one night’s accommodation is required to guarantee your reservation. Your room account must be paid in full to the hotel upon departure. Your hotel may require credit card as a guarantee at check-in. You are advised to return the Hotel Accommodation, Tours and Excursions Registration Form as early as possible. Rooms will be allocated on a first-come first-served basis and you will receive written confirmation of your reservation from GUARANT International. Any reservation made after June 23, 2005 will be subject to availability. Payment The following payment methods are accepted: Credit Card: For payment by credit card, please fill in the cardholder information on the Form. Please note that only Mastercard/Eurocard, VISA, Diners Club and American Express credit cards are accepted. Please include the Card Validation Code (CVC) these are the last three digits printed on the reverse side of your credit card in the signature field. The Accommodation prices are stated in Euros. However, credit card payments will be charged in Czech Koruna (CZK), your credit card company will convert this cost to the currency of your credit card. At the time of going to print the exchange rate is approximately 26 Czech Koruna = 1 Euro. Cancellation Conditions Up to 20 June 2005 full refund, less 25 EUR administration fee After 20 June 2005 1 night accommodation charge cancellation fee No Show 1 night accommodation charge cancellation fee In case of no shows, the room will be kept for the guest until 1200 hours on the day following the date of expected arrival; the room will then be released. Amendments Please inform GUARANT International of any amendments or cancellations, in writing by fax or e-mail. Telephone amendments will not be accepted. Please do not contact the hotel directly for any reservation changes or cancellations.
